USE OF PERSONAL INFORMATION
In our online registration form, we require you to provide us with your full name,
street address, city, zip code, phone number, email address, name on credit card
and credit card number, type and expiration date, username and password.
In the course of using our site, we automatically track certain information about
you. This information includes the URL that you just came from (whether this URL
is on our site or not), which URL you next go to (whether this URL is on our site
or not), what browser you are using, and your IP address. Many sites automatically
collect this information.
In addition, we use cookies on certain pages of our site. Cookies is simply an encrypted
file that allows a web site to store information about you on your computer. Among
other reasons, cookies allow you to enter your password less frequently during a
session. Cookies are stored on your hard drive, not on our site. Most cookies are
"session cookies", meaning that they are automatically deleted at the end of a session.
You are always free to decline our cookies if your browser permits, although in
that case you may be required to reenter your password more frequently during a
session.
If you send us personal correspondence, such as emails or letters, we may collect
such information into a file specific to you.
We may use your email address and/or your mailing address or phone number, to contact you
regarding administrative notices from time to time.
We do not sell, rent or divulge any information about you to any third party. This
includes information such as your password, and credit card number. We will release
specific information concerning your account only to comply with valid legal processes.
If you change credit cards, you may change it within your account information. To
change your credit card information, you must first key in the last four digits
and the expiration date of the card you are currently using. Upon your request,
we will remove your credit card number and credit card expiration date from our
databases. By doing so, we will cancel your subscription to our service. Reapplication
will entail new registration fees. To make this request, email us at
epinadmin@chesco.org.
You are encouraged not to reveal your password to any third parties or share it
with any third parties. You will be liable for all charges to your account arising
from misuse of access passwords. If your password has been compromised for any reason,
it should be changed immediately.
